We have streamlined the order process and forms in order to assure that you will receive the merchandise you order as quickly as possible. Please use only these new forms when placing orders with Dignity U Wear. Enter the appropriate password above to access the form(s) you need to download and complete; as we periodically update the forms, please access and download current forms with each new order.  An “Order Cover Sheet and Summary” form must accompany all orders.
